The Central Maine Medication Management Clinic focuses specifically on monitoring and dosing of Warfarin (Coumadin®), as well as other anticoagulant medications. Our clinical staff are specially trained in anticoagulation therapy, and will work closely with primary care providers to help patients manage their Warfarin/Coumadin dosing and reduce side effects and complications during therapy. A referral is required in order to establish care with our clinic. As part of this service, patients can expect to receive the following: Routine monitoring of Coumadin by certified anticoagulation nurse practitioners and RN's. The option for INR finger-sticks with time-of-service dosing at the Medication Management Clinic. The option for venipuncture at the Medication Management Clinic or lab of choice. The option for home INR finger-stick testing depending on PT's preference, eligibility, and insurance coverage. The clinic also provides perioperative bridging with Lovenox® and education consults for patients who may switch to novel oral anticoagulant (NOAC) medications, such as Eliqios®, Pradaxa®, and Xarelto®.
