Fryeburg Family Medicine is a designated Patient Centered Medical Home located on the Western Maine - New Hampshire border. We serve patients from several Maine counties and New Hampshire. We are accepting new patients of all ages and accept most insurances. As a fifth-year participant of the Maine Quality Counts - Patient Centered Medical Home pilot project we are working hard to find innovative ways to collaborate care, reduce cost, improve quality, and deliver your care in a safe, reliable, and trustful environment in a compassionate way. Our goal is to promote wellness and address healthcare needs that matter to you. Please join us in creating a new model of care delivery. Fryeburg Family Medicine offers the following special services: Phlebotomy on site Department of Transportation physicals Collaborative nurse calls Minor procedures Integrated behavioral healthcare Pediatrics, including newborns and geriatric care Osteopathic manipulation Our practice has been recognized by the Patient-Centered Medical Home Recognition Program and National Committee for Quality Assurance.
